Hematogenous metastases of hepatocellular carcinoma (HCC) to the small intestine are rare. In most cases, hematogenous metastasis is symptomatic, including perforation or bleeding, and the prognosis is poor. A 63-year-old man had lower right abdominal pain, and small intestinal obstruction due to chronic inflammation of the foreign body was suspected on computed tomography.

Introduction: Minimally invasive liver surgery (MILS), such as laparoscopic and robotic hepatectomy, has been developed and is an effective alternative to traditional open hepatectomy. Although surgical techniques and postoperative management have improved, many institutions continue to perform routine postoperative abdominal drainage. In open hepatectomy, abdominal drainage after uncomplicated hepatectomy increases overall and wound-related complications without a reduction in the risk of intra-abdominal fluid collections that require intervention.

Background: Crohn's disease (CD) is an incurable inflammatory condition that often requires multiple surgeries, negatively impacting quality of life. As such, treatment strategies that aim to prevent damage to the bowel and reduce the burden of surgeries for patients with CD are important. This retrospective, long-term, observational study investigated whether tumor necrosis factor inhibitor (TNFi) treatment was associated with decreased rates of abdominal surgery in Japanese patients with CD.

Idiopathic multicentric Castleman disease (iMCD) accompanied by thrombocytopenia, anasarca, fever, reticulin myelofibrosis/renal insufficiency, and organomegaly (TAFRO) frequently develops into prolonged severe thrombocytopenia and multiorgan dysfunction, which is associated with a high mortality rate. We herein describe three cases of iMCD-TAFRO in which serum soluble interleukin-2 receptor (sIL-2R) levels varied in relation to disease activity following the initiation of treatment. Thrombocytopenia persisted for several weeks to months after the normalization of the patients' C-reactive protein levels, whereas their sIL-2R levels normalized 10-47 days prior to platelet recovery (≥100×10/L).

Article Synopsis
  • This study focused on the factors that affect the visibility of early gastric cancer during endoscopic procedures and how linked color imaging compares to traditional white light imaging.
  • A total of 97 lesions were evaluated, with findings indicating that smaller lesions and the presence of intestinal metaplasia decreased visibility in white light imaging.
  • The results showed that linked color imaging significantly improved visibility scores, especially for smaller lesions and those with intestinal metaplasia, thereby suggesting it is a more effective imaging technique for detecting early gastric cancer.

Background And Aims: The impact of hepatitis C virus (HCV) eradication via direct-acting antiviral (DAA) therapy on overall mortality, particularly non-liver-related mortality, is understudied.

Methods: We recruited 4180 patients with chronic HCV infection who achieved sustained virological response (SVR) (HCV eradication) through DAA therapy (n = 2501, SVR group) or who did not receive antiviral therapy (n = 1679, non-SVR group); 1236 from each group were chosen using propensity score matching. Causes of death and all-cause mortality, including non-liver-related diseases, were investigated.

Article Synopsis
  • - This study explores the effectiveness of combining immune checkpoint inhibitors durvalumab and tremelimumab (DT) for patients with hepatocellular carcinoma (HCC) who previously received treatments like atezolizumab plus bevacizumab (AB).
  • - Results showed objective response rates (ORRs) were highest in first-line treatment (44%), followed by second-line without prior AB (54%), and significantly lower in patients who had prior AB (5%), indicating that prior treatment heavily affects success.
  • - The findings suggest that while DT may be less effective after AB treatment, it can still yield similar results to first-line therapy in patients without prior AB, highlighting the importance of treatment history when planning HCC therapy.
