Hematogenous metastases of hepatocellular carcinoma (HCC) to the small intestine are rare. In most cases, hematogenous metastasis is symptomatic, including perforation or bleeding, and the prognosis is poor. A 63-year-old man had lower right abdominal pain, and small intestinal obstruction due to chronic inflammation of the foreign body was suspected on computed tomography.

Introduction: Minimally invasive liver surgery (MILS), such as laparoscopic and robotic hepatectomy, has been developed and is an effective alternative to traditional open hepatectomy. Although surgical techniques and postoperative management have improved, many institutions continue to perform routine postoperative abdominal drainage. In open hepatectomy, abdominal drainage after uncomplicated hepatectomy increases overall and wound-related complications without a reduction in the risk of intra-abdominal fluid collections that require intervention.

We herein present a case involving a 62-year-old woman with a 1-month history of persistent dry cough. A thoracic computed tomography scan revealed multiple cystic lesions with well-defined thin septal thickening, tiny nodules, and areas of irregular consolidation characterized by irregular septal thickening in the right lower lobe. This thickening formed pentagonal and hexagonal patterns resembling a soccer ball.

Introduction: Clinically relevant postoperative pancreatic fistula (CR-POPF) after pancreaticoduodenectomy (PD) is one of the most life-threatening complications. Early drain removal after PD is recommended in the clinical setting; however, the risk stratification for CR-POPF to guide drain removal is not widely recognized.
Methods: This single-center retrospective study included patients who underwent PD between 2013 and 2024.

Background/purpose: The remnant stomach after distal gastrectomy (DG) which receives its blood supply mainly from the splenic artery (SPA), is at high risk for gastric ischemia following distal pancreatectomy (DP). We investigated the risk factors for ischemic necrosis of the remnant stomach (INS) during or after DP in DG patients.
Patients/methods: We collected 414 patients who underwent DP after DG between July 2009 and December 2019 by distributing questionnaires to members of the Japanese Society of Pancreatic Surgery (JSPS) in 2020, and the risk factors for INS were analyzed in 364 eligible patients.

Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a clinically diverse, demyelinating, and neuroinflammatory disorder of the central nervous system. We herein describe a case of MOGAD manifesting as limbic encephalitis (LE), characterized by a unilateral medial temporal lesion on magnetic resonance imaging of the brain. Electroencephalography (EEG) revealed sharp waves and occasional slow-wave activity in the bilateral parietal and occipitotemporal regions.

Idiopathic multicentric Castleman disease (iMCD) accompanied by thrombocytopenia, anasarca, fever, reticulin myelofibrosis/renal insufficiency, and organomegaly (TAFRO) frequently develops into prolonged severe thrombocytopenia and multiorgan dysfunction, which is associated with a high mortality rate. We herein describe three cases of iMCD-TAFRO in which serum soluble interleukin-2 receptor (sIL-2R) levels varied in relation to disease activity following the initiation of treatment. Thrombocytopenia persisted for several weeks to months after the normalization of the patients' C-reactive protein levels, whereas their sIL-2R levels normalized 10-47 days prior to platelet recovery (≥100×10/L).

Background: Several studies found that early pancreatic atrophy detected by computed tomography (CT) within 6 months was associated with a high incidence of diabetes in patients with type-1 autoimmune pancreatitis (AIP) receiving steroid therapy; however, no long-term follow-up studies have been performed.
Aim: To investigate pancreatic volume (PV) changes using three dimensional (3D)-CT volumetry and their relationship with IgG4 and diabetes in patients with AIP.
Methods: This retrospective study included 33 patients with type-1 AIP receiving steroid therapy.

Background And Aims: The impact of hepatitis C virus (HCV) eradication via direct-acting antiviral (DAA) therapy on overall mortality, particularly non-liver-related mortality, is understudied.
Methods: We recruited 4180 patients with chronic HCV infection who achieved sustained virological response (SVR) (HCV eradication) through DAA therapy (n = 2501, SVR group) or who did not receive antiviral therapy (n = 1679, non-SVR group); 1236 from each group were chosen using propensity score matching. Causes of death and all-cause mortality, including non-liver-related diseases, were investigated.

A 76-year-old man underwent distal pancreatectomy with celiac axis resection (DP-CAR) after preoperative chemotherapy for pancreatic cancer with celiac artery invasion. Although postoperative pancreatic leakage and ischemia-induced bile fistula developed, the patient's condition remained stable with good drainage. On postoperative Day 47, a pseudoaneurysm developed at the junction of the gastroduodenal artery and proper hepatic artery.
