The Nautilus intrasaccular system (EndoStream Medical, Israel) is a spiral-shaped neck-bridging endovascular device designed to support coiling of intracranial aneurysms [1-3]. It is deployed into the aneurysm sac through a 0.0165" or 0.017" microcatheter. Coils are delivered via a second microcatheter passed through the struts of the device. The system is available in 4-7-mm sizes and is entirely radio-opaque. It is mechanically detached and fully resheathable. Constructed from a nitinol wire with a platinum-based coil sleeve, it lacks a proximal marker. It has the lowest profile of current intrasaccular devices but a limited size range. Antiplatelet therapy is not mandatory. Limited published series and post-market data suggest high occlusion rates with low complication rates. The device may be particularly suitable for ruptured wide-necked aneurysms. This technical video discusses the device with the aid of two illustrative cases.

The treatment of wide-neck intracranial aneurysms (WNIAs) is a challenge in interventional therapy. Intrasaccular therapy, including intrasaccular flow disruption or formation of a neck bridge within the saccular, has been extensively used recently. Purpose: Various adjunctive devices have been reported to be helpful during the endovascular coiling of complex, wide-necked cerebral aneurysms. This study aimed to describe the utilization of a recently introduced intrasaccular neck-bridging implant specifically designed to facilitate coil embolization.
Methods: We retrospectively reviewed eight consecutive patients who underwent treatment for intracranial aneurysms with the Nautilus intrasaccular bridging system between February 2022 to May 2022.

Numerous devices and sophisticated strategies have been developed to further increase the number of aneurysms amenable to endovascular treatment.1-4 Despite the superfluity of available neurovascular armamentarium, wide-necked bifurcation aneurysms can still pose a significant technical challenge to the treating clinician.5-7 Neck bridging is a conceptually new approach, which provides increased occlusion rates with lower recurrence and complications rates.
