Background: Amyloidosis is a disorder characterized by misfolded protein deposits in organs, often manifesting as cardiac disease.
Case Summary: A 60-year-old male with a history of isolated proteinuria and recent fat biopsy-proven transthyretin (TTR) amyloidosis was referred to us for evaluation of cardiac involvement with amyloidosis. He underwent a technetium pyrophosphate scan which showed Perugini grade 3 uptake concerning for cardiac involvement. His persistent proteinuria and serum IgG kappa monoclonal protein raised suspicion for light-chain (AL) amyloidosis. Endomyocardial biopsy confirmed TTR cardiac amyloidosis, while a subsequent kidney biopsy and bone marrow evaluation revealed renal-limited AL kappa amyloidosis.
Discussion: This rare presentation highlights the challenge of distinguishing between TTR and AL amyloidosis, especially with atypical features. It underscores the necessity for tissue diagnosis and mass spectrometry in complex cases, even with seemingly concordant noninvasive findings.

Background: We present the case of a 74-year-old woman diagnosed with obstructive hypertrophic cardiomyopathy.
Case Summary: Amyloidosis was initially considered because she was genotype positive in the transthyretin gene. However, because of 2 negative 99m technetium pyrophosphate radionuclide scans, this diagnosis was considered unlikely, and endomyocardial biopsy was deferred.

The mechanisms driving amyloid assembly have long intrigued structural biologists, as they offer insights into systemic fibrotic changes and the dynamic behavior of transthyretin (TTR) aggregation, crucial for developing amyloid-targeted therapies. In TTR-associated amyloidosis, amyloid fibrils form via destabilization of the tetramer into dimers and monomers. While many TTR mutations have been studied, the atomistic impact of multiple mutations on amyloid transthyretin (ATTR) self-assembly remains underexplored.

Hereditary transthyretin (ATTRv) amyloidosis is a progressive multisystem disorder, mainly characterized by cardiac dysfunction and polyneuropathy. Due to its rarity and heterogeneous presentation, diagnosis is often delayed, which has a direct impact on the initiation of treatment and, therefore, span and quality of life. To facilitate early disease recognition, we aimed to develop and validate a new screening tool for early identification of ATTRv amyloidosis with polyneuropathy (AmyloScan).

Background: The treatment and prognosis of cardiac amyloidosis (CA) depend heavily on the accurate identification of amyloid protein types. Histopathological methods are the most commonly used approach, but often produce inconclusive results. The application of mass spectrometry with laser microdissection mass spectrometry based on non-targeted proteomics in CA diagnosis is gradually being recognized, but it is expensive, time-consuming, and still in the early stages of scientific research applications.
