This case report describes, to our knowledge, the first known successful treatment of airborne allergic contact dermatitis to sesquiterpene lactone with upadacitinib in an adult patient. Recurrent summertime photodistributed eczematous eruptions should raise suspicion for airborne allergic contact dermatitis to sesquiterpene lactone, and positive patch test results to sesquiterpene lactone and Compositae in these patients should be considered highly relevant. In addition to allergen avoidance, treatment with systemic Janus kinase inhibitors can induce allergic contact dermatitis remission.

Rationale: Immune-checkpoint inhibitors have emerged as a frontline treatment for a growing list of malignancies. Immunotherapy-induced bullous pemphigoid (BP) is a rare dermatological immune-related adverse event of immune-checkpoint inhibitor therapy immune-checkpoint inhibitor therapy. We report a case of immunotherapy-associated BP, with lesions presenting in a photodistribution.

While topical steroids are an invaluable tool used for the treatment of pruritic rashes, they can also complicate attempts at a definitive diagnosis. The temporal association between steroid use and biopsy must be taken into account when investigating the underlying etiology of such rashes. In patients with a history of dermatologic disease with the onset of a new rash, management should include a biopsy, if deemed necessary for diagnosis, followed by a prescription of topical steroids for symptomatic treatment.
