Dural opening and closure represent a crucial yet under-discussed aspect of transsphenoidal surgery (TSS), where the operative field is narrow and cerebrospinal fluid (CSF) leakage remains a significant complication. In 2010, we abandoned the conventional cruciate dural incision and began developing optimized designs that provide wide exposure, minimal interference, and facilitate watertight closure. After investigating various incision types, we empirically established a modified H-shaped dural incision in 2011.

Quadrupolar-perturbed solid-state NMR spectroscopy is a highly useful and well-established method for studying quadrupolar nuclei. This method relies on a high ratio of the Larmor frequency to the quadrupolar frequency and is limited, therefore, by the available magnetic field strengths suitable for NMR, which are on the order of 10 T. Nuclear quadrupole resonance (NQR) provides an approach to studying strongly quadrupolar isotopes, but there are technical challenges associated with measuring high-frequency transitions, and with measuring both the quadrupolar coupling constant, C, and asymmetry parameter, η, with good precision.

Majority of commercial L-asparaginase (L-ASNase) activity assays are based on coupled enzymatic reaction, which converts aspartate into pyruvate, subsequently reacting with the probe to form a stable chromophore, which can be detected spectrophotometrically. However, in complex biological samples this method can be inaccurate due to poor optical transparency or presence of compounds interfering with the coupled enzyme reaction-for this kind of cases alternative methods have been suggested. Here we suggest a strategy to rationally pick a method of choice in a variety of situations, taking into consideration the upsides and downsides of each method.

Introduction: Sleeve gastrectomy (SG) is the most performed bariatric surgery in the US, with a small but growing trend toward outpatient surgery. The aims of this study are to (1) clarify the safety of sleeve gastrectomy with same-day discharge (SDDSG) vs sleeve gastrectomy with inpatient stay (SGIS) utilizing recent MBSAQIP data, (2) identify specific predictors of complication in patients undergoing SDDSG, and (3) understand the relative uptake of SDDSG among the population for whom it would be low-risk.

Methods: This is a retrospective cohort study of the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP) database.
