This case highlights the intersection of psychiatric symptoms and hypermobility spectrum disorder (HSD) in a woman in her 20s with multiple comorbidities, including postural orthostatic tachycardia syndrome, chronic pain and obstructive sleep apnoea. The patient had been unresponsive to conventional psychiatric treatments for anxiety and panic attacks. On recognition of HSD, a multidisciplinary approach, including propranolol for autonomic dysfunction, pregabalin for pain and anxiety, and lisdexamfetamine for cognitive impairment, significantly improved her condition.

Objectives: Surges in demand for sedatives for mechanical ventilation during the COVID-19 pandemic caused shortages of sedatives globally. Propranolol, a nonselective beta-adrenergic blocker, has been associated with reduced agitation and sedative needs in observational studies. We aimed to test whether propranolol could reduce the dose of sedatives needed in mechanically ventilated patients.

Background: Beta-blocker prescriptions for patients with anxiety increased substantially between 2003 and 2018, yet there is no clinical guidance concerning their use. A previous review of propranolol - a beta-blocker - in the treatment of anxiety concluded there was insufficient evidence to support its use. Additional data have been published in the eight years since that review including some evidence for other beta-blockers.

Exposure therapy is an evidence-based treatment option for anxiety-related disorders. Many patients also take medication that could, in principle, affect exposure therapy efficacy. Clinical and laboratory evidence indeed suggests that benzodiazepines may have detrimental effects.
