Article Abstract

Background: Cutibacterium acnes (C. acnes), a gram-positive bacterium commonly found in the pilosebaceous glands of the shoulder, represents a significant source of infection following shoulder surgery. Identifying patients at higher risk for bacterial contamination prior to surgery could facilitate the implementation of enhanced prophylactic measures. The objective of this study was to identify biomarkers and related factors that predict the detection of C. acnes at the portal of arthroscopic shoulder surgery.

Methods: This study included 110 patients undergoing arthroscopic shoulder surgery. Swab cultures were obtained from the arthroscopic portal approximately 1 h post-creation to assess for the presence of C. acnes. Preoperative inflammatory biomarkers, including the neutrophil-to-lymphocyte ratio (NLR) and monocyte-to-lymphocyte ratio (MLR), were evaluated in relation to bacterial detection.

Results: C. acnes was identified in 18.2 % of patients, with all positive cases exclusively observed in male patients. Both NLR and MLR were significantly elevated in patients with positive C. acnes cultures. Receiver operating characteristic (ROC) curve analysis identified optimal threshold values of 1.73 for NLR and 0.17 for MLR in predicting C. acnes detection.

Conclusion: Elevated preoperative NLR and MLR are associated with the intraoperative detection of C. acnes at the arthroscopic portal site. These readily available biomarkers may help identify patients at increased risk for bacterial contamination, informing preoperative prophylactic protocols.

Purpose: This study aimed to compare clinical outcomes between open and arthroscopic anterior latissimus dorsi (LD) transfer techniques for treating irreparable subscapularis (SSC) tears.

Methods: We retrospectively reviewed patients who underwent open or arthroscopic anterior LD transfer for irreparable SSC tears between February 2014 and August 2020. Patients were included if they had irreparable SSC tears with Lafosse Grade 4 or higher and Goutallier Grade 3 or higher, but without advanced arthritis (Hamada Grade < 3).

» Kim's lesion of the shoulder is characterized by incomplete tearing of the junction between the posteroinferior labrum and the glenoid, with the superficial labral tissue remaining intact, and generally requires arthroscopic evaluation for accurate confirmation.» Kim's lesion represents an under-reported subtype of posterior labral injury and a source of activity-related posterior shoulder discomfort and instability.» Kim's lesions are frequently observed in young, active individuals involved in overhead and contact sports, often resulting from traumatic mechanisms with the shoulder in flexion and adduction, as well as from repetitive microtrauma and overuse.

Background: Tranexamic acid (TXA) is an anti-fibrinolytic agent that has effectively reduced transfusion risk and minimized blood loss after total joint arthroplasty. TXA use has had mixed results on postoperative pain after arthroscopic rotator cuff repair (ARCR). The purpose of this prospective, double-blind, randomized, controlled trial was to examine the impact of TXA on prescription opioid consumption for three postoperative days in an outpatient population after ARCR.

Background: Shoulder instability is a frequent concern in active duty military personnel because of the high physical demands of service. Arthroscopic Bankart repair is commonly employed to address anterior shoulder instability, but there is limited data on return-to-duty outcomes in military cohorts, particularly in the South Korean population.

Methods: This retrospective study included 30 active duty male soldiers who underwent isolated arthroscopic Bankart repair at a single military hospital.
