Background: The heart failure with preserved ejection fraction (HFpEF) scores were initially developed to facilitate diagnosis of HFpEF in patients with dyspnea. Whether these diagnostic scores also relate to severity of cardiopulmonary reserve limitations across the broad range of patients with dyspnea, with or without confirmed HFpEF, remains unclear.

Obesity is a major risk factor for heart failure with preserved ejection fraction (HFpEF) and is associated with a distinct pathophysiological phenotype. Individuals with obesity develop HFpEF on average a decade earlier than individuals without obesity. Despite this, systematic screening for HFpEF in individuals with obesity is not currently recommended in clinical guidelines.

Heart failure with preserved ejection fraction (HFpEF) is under-recognized in clinical practice. Although a previously developed risk score, termed HFPEF, can be used to estimate HFpEF probability, this score requires imaging data, which is often unavailable. Here we sought to develop an HFpEF screening model that is based exclusively on clinical variables and that can guide the need for echocardiography and further testing.
