An untargeted transcriptomic analysis provided exploratory insight into key genes and pathways that can be used to help identify patients at risk for surgically refractory chronic rhinosinusitis with nasal polyps (CRSwNP). The Charcot-Leyden crystal gene, periostin and osteopontin-associated pathways, eicosanoid synthesis pathway, and prostaglandin/leukotriene metabolism pathway, among others, are associated with dupilumab prescription for surgically refractory chronic rhinosinusitis and may support early stratification of disease severity in CRSwNP patients.

The introduction of autologous neurosensory retinal transplantation (ANRT) into vitreoretinal surgery has significantly improved the success rates of closure of refractory full-thickness macular holes (FTMH). In recent years, the technique has gained wide acceptance and its indications have expanded; however, certain aspects remain debatable - particularly the optimal graft size to ensure the best anatomical and functional outcomes. To address this issue, the study proposes a surgical technique for treating FTMH using ANRT that involves precise marking of the neurosensory retinal graft.

Metastatic and relapsed osteosarcoma (OS) remains difficult to treat despite advanced surgical techniques, intensified chemotherapy, and targeted therapies. Adoptive immunotherapies such as chimeric antigen receptor (CAR) T cells, are in their nascent stage, but remain a viable therapeutic strategy for patients with aggressive solid tumors such as OS. Folate receptor- (FOLR1) has been functionally implicated in OS pathophysiology, providing rationale as a potential therapeutic target.

Thoracic duct embolization has emerged as an alternative treatment for refractory chylothorax, in addition to thoracic duct ligation. Thoracic duct embolization is typically performed via direct puncture of the cisterna chyli and cannulation of the thoracic duct, or via a retrograde approach to the thoracic duct through the venous angle. The former requires a long puncture through abdominal organs; the latter has anatomical limitations depending on the case.

Purpose: To report two cases of macular exudations resulting from retinal arterial macroaneurysms (MaAs) refractory to focal photocoagulations that were treated with a new surgical technique including subretinal balanced saline solution (BSS) injection to dilute lipid-rich subretinal fluid (SRF) and facilitate absorption of the SRF, intentional retinal hole formation to direct SRF into the vitreous cavity, and laser photocoagulation posterior to the MaAs to prevent intraretinal fluid and SRF from reaching the fovea.
Observations: A 70-year-old man with macular edema (ME) refractory to anti-vascular endothelial growth factor (VEGF) therapy was referred to our hospital. Fundus examination showed retinal arterial MaAs and hard exudations.
