Hilar cholangiocarcinoma (HCCA) is a rare, aggressive cancer often diagnosed at an unresectable stage. Patients commonly require systemic therapy and biliary stenting to manage symptoms and maintain liver function. Histotripsy is a novel, non-invasive, mechanical ablation technique recently FDA-approved for liver tumors. We report the case of a 77-year-old woman with recurrent HCCA who experienced progressive biliary obstruction despite chemotherapy and irreversible electroporation. She underwent 2 staged histotripsy treatments targeting tumors in the left and right biliary systems. Procedures were well-tolerated, with no complications, and led to normalization of bilirubin within 72 hours. Imaging at 6 months showed stable disease and sustained biliary patency, with only 1 stent exchange needed. This case highlights the potential role of histotripsy in relieving biliary obstruction and controlling local disease in patients with HCCA.

Intussusception is an uncommon cause of intestinal obstruction in adults and rarely encountered during pregnancy. Duodenal intussusception is particularly rare due to the fixed position of the duodenum. We report a unique case of duodenojejunal intussusception in a pregnant woman at 28 weeks of gestation, who presented with symptoms mimicking acute pancreatitis complicated by biliary tract obstruction.

Biliary radiofrequency ablation is an emerging adjunctive and palliative therapy for patients with ampullary and biliary tumors. Given the high mortality for these malignancies, data on long-term complications are limited. We report a unique case of sphincterotomy restenosis causing biliary obstruction in a 98-year-old woman with a history of ampullary adenocarcinoma treated with papillectomy and biliary radiofrequency ablation (RFA).

This case report presents a complex case of acute cholecystitis, cholangitis, pancreatitis, intrahepatic abscesses, and sepsis without biliary obstruction, highlighting the challenges of managing multi-organ involvement in a critically ill individual. The patient, a middle-aged male, presented with fever, jaundice, and abdominal pain, with imaging revealing biliary ductal dilation, a distended gallbladder, and a staghorn calculus. Laboratory findings showed elevated liver enzymes, bilirubin, and lipase, supporting the diagnosis of acute cholecystitis, cholangitis, and pancreatitis.

Duodenal perforation is a rare but harmful complication of endoscopic retrograde cholangiopancreatography (ERCP). Early diagnosis and appropriate management are critical to reduce morbidity and mortality.
