Article Abstract

This video demonstrates our surgical approach for an entirely tubular trans-spinous process bilateral lumbar discectomy. A 47-year-old man presented with 12 months of severe predominantly right L5 radiculopathy. Magnetic resonance imaging revealed a large diffuse disc bulge at L4/5 resulting in bilateral lateral recess and central stenosis. He underwent bilateral discectomies via a trans-spinous process tubular approach. Postoperatively, his pain resolved and was discharged home the following day. Clinical resolution was sustained at 6 weeks. An entirely tubular trans-spinous process technique for lumbar decompression provides excellent bilateral lateral recess exposure through a minimally invasive muscle-sparing approach.

Aim: To evaluate whether adding an extra anchoring point to the construct by passing the crosslink through a hole in the spinous process (trans-spinous crosslink technique), may prevent screw loosening by increasing the pull-out strength.

Material And Methods: Twenty-four fresh-frozen single lumbar sheep vertebrae were instrumented with pedicle screws bilaterally, and they are connected to each other with a crosslink. All vertebrae were assigned randomly to either the experiment (trans-spinous crosslink) group or the control group.

An in vitro experiment was designed to evaluate the mechanical properties of a new trans spinous process wiring method (KMC method) for segmental spinal instrumentation by using the goat spine model and double Luque L rods. Specimens were tested by a universal testing machine with the cyclic 3-point bending load. Instrumentation with Wisconsin trans spinous process button & wire system was also tested for comparison.
