Dulaglutide, a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist, is approved for improving glycemic control and reducing cardiovascular risks in patients with type 2 diabetes mellitus (T2DM). This research investigates the effect of dulaglutide on gastric emptying and its impact on the pharmacokinetics (PK) of orally administered molecules utilizing a combination of population pharmacokinetic (PopPK) and physiologically based pharmacokinetic (PBPK) modeling approaches. In clinical studies, the gastric emptying delay (GED) was evaluated in healthy participants and patients with T2DM at various dose levels of dulaglutide. A PopPK model estimated the exposure-dependent delay in gastric emptying, which was then input into the orally administered small molecule PBPK models. These PBPK models, informed by internal clinical studies and publicly available data, quantified the effect of dulaglutide-induced GED on the area under the curve (AUC), maximum concentration (C), and time to maximum concentration (t) of the co-administered drugs. The modeling approach was verified for reproducing observed GED-mediated drug-drug interactions (DDIs) at low doses of dulaglutide and to predict DDIs at a 4.5 mg dulaglutide dose. The clinical studies demonstrated that the 1.5 mg dulaglutide dose has no clinically relevant effect on the pharmacokinetics of small molecules, and the modeling led to a similar conclusion at 4.5 mg dulaglutide. This work demonstrates that modeling approaches can be used to predict potential GLP-1-mediated DDIs related to gastric emptying delay, increasing the efficiency of the clinical pharmacology programs.

Glucagon-like peptide 1 receptor agonists (GLP-1 RAs) have gained prominence for their efficacy in treating type 2 diabetes and obesity. Recent evidence suggests that their pleiotropic effects-beyond glycemic control and weight loss-include anti-inflammatory, immunomodulatory, and antioxidative effects, which may beneficially support various dermatologic conditions such as psoriasis, hidradenitis suppurativa, acanthosis nigricans, and Hailey-Hailey disease. However, GLP-1 RAs are also associated with emerging cutaneous adverse drug reactions, including bullous, exanthematous and vasculitic manifestations, and other rare side effects.

Background And Aims: Gastroesophageal reflux (GER) is common and thought to contribute to disease progression in patients with respiratory disease. Delayed gastric emptying (DGE) can increase GER in patients with GER disease, but its effect in patients with respiratory disease, and how differing lung structure (eg, scarring, inflammation) and mechanics (eg, decreased thoracic pressure in restrictive disease, increased abdominal pressure in obstructive disease) influences this is unknown. Dumping syndrome (DS) is a known complication following bariatric surgery, caused by rapid gastric emptying into the small intestine. It presents in two forms: early dumping, with gastrointestinal and vasomotor symptoms occurring within 30-60 min after meals; and late dumping, which arises 1-3 h postprandially due to reactive hypoglycaemia. Standard management includes dietary changes and medications, but tolerability and long-term efficacy are variable.

Objective: This systematic review and meta-analysis evaluates the efficacy of bilateral subthalamic nucleus (STN) deep brain stimulation (DBS) in improving gastric motility in patients with Parkinson's disease (PD).
Materials And Methods: A systematic search was conducted in Scopus, Web of Science, PubMed, and Cochrane CENTRAL until December 15, 2024. Observational studies and clinical trials evaluating DBS effects on gastric motility and gastric emptying in patients with PD were included.
