A 54-year-old man with autism presented to our hospital for the evaluation of anemia and body weight loss. The patient had mild upper right abdominal pain, and computed tomography revealed wall thickening of the duodenal bulb. Esophagogastroduodenoscopy revealed that a bread-bag clip (BC) pierced the pyloric ring and that a duodenal ulcer was present at the puncture site. It was endoscopically removed using grasping forceps. Ingestion of BCs often cause gastrointestinal perforation. We recommend that emergency endoscopy be performed when BC ingestion is suspected.
