A 61-year-old woman experienced a traumatic globe dislocation after falling onto a motorcycle handlebar. Initially, this extremely rare injury was overlooked because distracting facial injuries obscured its typical signs. A CT scan ultimately revealed that the eye had been displaced into the maxillary sinus. Although surgical repositioning was successful, the patient suffered irreversible vision loss due to damage to the optic nerve. This case serves as a critical reminder that in cases of severe facial trauma, urgent imaging is essential to rule out concealed ocular injuries, especially when direct examination is hindered by significant swelling or wounds.
