Neurofibromatosis type 1 (NF-1) is an autosomal dominant disorder associated with vascular abnormalities, including spontaneous hemothorax and arterial aneurysms. We present a rare case of spontaneous hemothorax in which an apparently hemostatic sub-pleural hematoma began to bleed again after the patient was repositioned. A 47-year-old man with NF-1 presented with the sudden onset of left-sided chest pain.

The David procedure has been extensively studied as an elective treatment when valve anatomy and function permit valve-sparing aortic root replacement. This approach is particularly beneficial in young patients who also require mitral valve repair and treatment for mitral annular disjunction. This video tutorial provides a step-by-step guide to the David V procedure in a female patient with Marfan syndrome, presenting with an aortic root aneurysm, severe mitral regurgitation and mitral annular disjunction.

Splenic artery aneurysms (SAA) are the most common visceral aneurysm and may develop significant risk for rupture and life-threatening complications when more than 3 cm, in pregnancy, or if resembling pseudoaneurysms. Traditional treatment options of open surgical repair and endovascular repair have been extensively studied, but the role of robotic-assisted laparoscopic surgery in definitively managing SAA is emerging as a promising minimally invasive alternative in select pathology. This case aims to present the technical details, challenges, and outcomes of an SAA in a young woman treated with robotic-assisted laparoscopic resection, highlighting the feasibility and efficacy of this approach.
