According to many criteria, implementation of lean technologies into medical organizations rendering primary health care is implemented through project activities. At that, experience of implementation of projects presents different results in terms of efficiency in clinics of the Moscow Oblast that is partly related to psychological characteristics of employees and unpreparedness for changes in work. The purpose of the study is to assess particular psychological characteristics of medical workers in primary health care organizations which may affect implementation of changes in basic processes in functioning of clinics. The questionnaire survey of 3 978 managers and workers of polyclinics of the Moscow Oblast was carried out concerning differential type of reflection, cognitive regulation of emotions and integral types of communicative aggressiveness to analyze psychological status of workers of polyclinics where project activities were implemented. According to differential type of reflection average level of systemic reflection is decreased in 62.6% of respondents. In 7% evidence of quasi-reflection indicator was revealed. In 20.78% of employees deviation in introspection parameter was detected. On cognitive regulation of emotions self-blame was above "norm" in 32.5% of respondents; indicator of rumination was above "norm" in 22.8% of workers, deviation from the "norm" in terms of catastrophizing was marked in 7.9%, blaming others in 4.5%, acceptance was decreased in 70.6% of respondents, positive refocusing was insufficient in 77.7% of workers of polyclinics, perspective consideration was insufficient in 65.8% of respondents; and 51.2% of respondents had weak focusing on planning. On integral types of communicative aggressiviness low level of communicative aggression was established. At that, in 85.9% of respondents parameter "inability to switch aggression to activities or inanimate objects" was above average value. To increase efficiency of results of implementation of changes within the framework of project activities it is necessary to carry out psychological work with personnel of medical organizations in individual and group (training) format.

While international and local clinical care guidelines and pathways have been formulated to optimize the prevention and treatment of DFUs, a continuous audit of real-world adherence among healthcare professionals (HCPs) is needed to ensure care quality, safety, and efficacy.

Background: Healthier SG (HSG), introduced in July 2023 by Singapore's Ministry of Health as a nationwide primary care reform initiative, aims to enhance preventive care within the primary care sector, strengthen patient-physician relationships, and integrate health and social care. It incorporates a transition to capitation-based payment models and introduces structured "health plans" within consultations. Given the significant role of primary care physicians in implementing HSG, understanding their perspectives - including their attitudes, as well as the facilitators and barriers they encounter - is critical for addressing operational challenges and ensuring the initiative's success.
