Article Abstract
We report the infrequent occurrence of the annular pancreas (AP) in adults by presenting two remarkable cases. The first case involved an incidental discovery of an AP in a patient diagnosed with chronic pancreatitis. Surgical intervention was warranted due to gastric outlet obstruction resulting from pyloric stenosis. The patient underwent loop gastrojejunostomy (GJ) and duodenojejunostomy (DJ). The second case was evaluated for dyspeptic symptoms, and the AP was diagnosed on the contrast-enhanced abdomen CT. A bypass procedure in the form of loop DJ was performed to address the obstruction. These two cases underscore the distinctive presentations of AP and highlight the need for tailored strategies in their management.

Multiseptate gallbladder (MG) is a rare congenital malformation that may occur as an isolated anomaly or in conjunction with other abnormalities of the pancreaticobiliary, urinary, and gastrointestinal systems. The coexistence of MG with an annular pancreas is an exceedingly rare phenomenon. The diagnosis is typically established using abdominal ultrasound; however, further imaging modalities are often required to delineate the detailed anatomy and to identify any associated anomalies within the pancreaticobiliary system.

Portal annular pancreas (PAP) is one of the rare pancreatic anomalies in which the pancreatic parenchyma surrounds the portal vein (PV) or superior mesenteric vein (SMV), accounting for only around a few proportions of all patients. PAP is thought to be associated with the high risk of postoperative pancreatic fistula (POPF) after pancreatectomy. We describe our experience of a case with PAP and review the literature on pancreatectomy in patients with PAP.

Usually, it is associated with obstruction of the second part of the duodenum, but to the authors' knowledge, this is the first reported case of the partial annular pancreas obstructing the first part of the duodenum, leading to late presentation of gastric outlet obstruction. As clinicians, we should be vigilant about the causes of abdominal fullness and pain and consider the annular pancreas an important differential diagnosis.

Polysplenia syndrome is a rare congenital anomaly characterized by visceral malposition associated with multiple spleens. In relation to the pancreas, reported abnormalities include the annular pancreas, as well as defects in the body, tail, or uncinate part of the pancreas.
