The widespread use of cardiac magnetic resonance imaging (MRI) in clinical practice has enabled the identification of numerous patients with evident damage from previous myocarditis, whether known or unknown. For years, myocardial fibrosis has been a topic of interest due to its established correlation with arrhythmic events in various clinical settings, including ischemic heart disease, dilated cardiomyopathy, and hypertrophic cardiomyopathy. MIBG scintigraphy is a method widely used in patients who are candidates for defibrillator implantation or have experienced heart failure.

A 58-year-old man with no significant medical history or prescribed medications, including coronary artery disease, presented to the emergency department with hypoxemia due to COVID-19. A non-contrast chest computed tomography (CT) ordered to evaluate for the cause of the patient's hypoxemia revealed mild ground-glass opacities and moderate incidental coronary artery calcium (iCAC), noted in the body of the report. The CT report impression only reads "Peripheral ground glass opacities in keeping with COVID-19 infection.

Background: Arachnoid cysts are mostly benign, cerebrospinal fluid-filled sacs within the arachnoid membrane. Although they are often asymptomatic and discovered incidentally, they can also present with serious complications such as hemorrhage or rupture.
