Background And Objectives: Iclepertin, a selective glycine transporter-1 inhibitor, was investigated as a potential treatment for cognitive impairment associated with schizophrenia. The objective of this trial was to determine the effect of food on the pharmacokinetics of iclepertin 10 mg.
Methods: This Phase I, open-label, 2-period cross-over trial randomised (1:1) healthy volunteers to 2 treatment sequences (fasted-fed or fed-fasted) to receive a single oral dose of iclepertin 10 mg once daily in either the fasted or fed state followed by cross-over to the other state. Primary endpoints included maximum measured concentration in plasma (C) and area under the concentration-time curve over the time interval from 0 to the last quantifiable data point (AUC). The secondary endpoint was area under the concentration-time curve over the time interval from 0 extrapolated to infinity (AUC). Relative bioavailability was estimated by calculating an adjusted geometric mean (gMean) fed/fasted ratio using analysis of variance. Safety was assessed.
Results: Of 16 participants enrolled [mean (SD) age: 37.1 (10.1) years], 15 were included in the analysis. The C, AUC and AUC of iclepertin were higher in the fed versus fasted state; adjusted gMean ratios (90% confidence intervals) were 118.33% (110.01, 127.28), 114.61% (110.13, 119.27) and 114.38% (110.11, 118.80), respectively. Iclepertin 10 mg was well tolerated.
Conclusion: Iclepertin exposure was higher in fed versus fasted conditions, but the increase was minor, suggesting food has no meaningful effect on the pharmacokinetics of iclepertin 10 mg.

Background: Iclepertin is a selective inhibitor of glycine transporter 1 recently investigated as a novel treatment for cognitive impairment associated with schizophrenia. Iclepertin is a potential mild inducer of liver cytochrome P450 3A4, which metabolises ethinylestradiol and levonorgestrel, which are used in combined oral contraceptives (OCs).
Objectives: This trial investigated the potential drug interaction effect of steady-state iclepertin on the steady-state pharmacokinetics of combined OCs.

Despite predicting poor functional outcomes and being a significant patient burden, there are no approved pharmacotherapies to treat symptoms of cognitive impairment associated with schizophrenia (CIAS). Iclepertin (BI 425809) is a potent and selective inhibitor of glycine transporter-1 (GlyT1) that was in Phase III development for the treatment of CIAS. Iclepertin is metabolized by the cytochrome P450 (CYP) 3A4 enzyme and also induces CYP3A4 at supratherapeutic concentrations, so drug-drug interactions (DDIs) with CYP3A4 perpetrators and substrates may be expected.

Iclepertin was a promising drug candidate being developed for the treatment of cognitive impairment associated with schizophrenia. Interestingly, the predominant major metabolite in vivo in humans, BI 761036 (M232), was not observed after incubations in hepatocytes or hepatic subcellular fractions. Subsequent studies revealed that the metabolic pathway leading to the formation of M232 involves 2 sequential oxidation steps.

Purpose: After single oral dosing of the glycine reuptake transporter (GlyT1) inhibitor, iclepertin (BI 425809), a single major circulating metabolite, M530a, was identified. However, upon multiple dosing, a second major metabolite, M232, was observed with exposure levels ~ twofold higher than M530a. Studies were conducted to characterize the metabolic pathways and enzymes responsible for formation of both major human metabolites.
