A 58-year-old male in cardiogenic shock requiring a left ventricular assist device and veno-arterial extracorporeal membrane oxygenation with worsening lactic acidosis was found to have acute occlusion of the visceral aorta, celiac axis, superior mesenteric artery (SMA), and left renal artery. We describe the off-label use of the Inari ProTrieve Sheath to prevent distal thromboembolic events during mechanical aortic thrombectomy in a patient on extracorporeal membrane oxygenation who could not tolerate aortic cross-clamping. Retrograde SMA thrombectomy and patch angioplasty was subsequently performed, followed by temporary abdominal closure. The patient's lactic acidosis improved significantly, and repeat imaging revealed a patent aorta and SMA.

Endovascular surgeries generally rely on push-based catheters and guidewires, which require significant training to master and can still result in high stress being exerted on the anatomy, especially in tortuous paths. Because these procedures are so technically challenging to perform, many patients have limited access to high-quality treatment. Although various robotic systems have been developed to enhance navigation capabilities, they can also apply high stresses due to sliding against the vascular walls, impeding movement and raising the risk of vascular damage.

Chorea after cardiac surgery (formerly post-pump chorea) is a common complication in children (estimated prevalence of 0.6%-3%) but is rare in adults; when it does occur, it is often permanent and less responsive to treatment. A 47-year-old woman developed chorea in all limbs following cardiac surgery for a pseudoaneurysm of the anastomosis along with thrombectomy of the prosthetic aortic valve (after Bentall's procedure in the past).

Aim: To evaluate treatment options and outcomes in a series of hyperacute presentations of intracardiac, aortic or arterial thrombosis in neonates.
Methods: All neonates with proven aortic or arterial thrombus without iatrogenic aetiology were reviewed retrospectively in a single-centre series from January 1990 to September 2023.
