Head and neck paragangliomas are rare; among these, glomus jugulare is the second most common. Due to its rarity, close proximity, and adhesion to vital neurovascular structures, treatment of paraganglioma is challenging. Surgery remains the treatment of choice; however, in scenarios of inoperability, molecular theranostics can aid patient management. This case highlights the role of dual-tracer positron emission tomography-computed tomography (PET-CT) imaging, that is, fluorodeoxyglucose (FDG) and somatostatin receptor (SSTR)-based PET-CT imaging with ⁶⁸Gallium-DOTATATE, aiding treatment planning with targeted ¹⁷⁷Lutetium-DOTATATE therapy.
