Necrotizing pancreatitis represents a life-threatening sequela of acute pancreatitis in which rapid necrotization of pancreatic and abdominal tissue leads to inflammation and sometimes infection. Treatment includes debridement of necrotic tissue to prevent inflammation from spreading and antibiotics if infected. Areas of necrosis are accessed through percutaneous drainage, surgical debridement, or upper endoscopy.

Background And Aims: EUS-guided pelvic abscess drainage (EUS-PAD) with a lumen-apposing metal stent (LAMS) is an alternative approach for postsurgical and inflammatory pelvic collections. This study assessed the safety and efficacy of EUS-PAD with an LAMS in a large population of complicated acute diverticulitis (AD).
Methods: Data were retrieved on consecutive patients who underwent EUS drainage attempts for complicated AD from 2019 to 2023 in 3 referral centers.

A 53-year-old woman with a history of laparoscopic appendectomy for acute appendicitis 20 years ago presented to our endoscopy center for colonoscopy surveillance and found a circumferential Paris 0-IIb laterally spreading lesion in the appendix orifice. After indigo carmine mixed saline staining, the lesion showed a type II-open pit pattern, which grew into the appendiceal residual lumen. The single-channel transcolonic endoscopic appendectomy (TEA) was performed.
