Purpose: To report two cases of macular exudations resulting from retinal arterial macroaneurysms (MaAs) refractory to focal photocoagulations that were treated with a new surgical technique including subretinal balanced saline solution (BSS) injection to dilute lipid-rich subretinal fluid (SRF) and facilitate absorption of the SRF, intentional retinal hole formation to direct SRF into the vitreous cavity, and laser photocoagulation posterior to the MaAs to prevent intraretinal fluid and SRF from reaching the fovea.
Observations: A 70-year-old man with macular edema (ME) refractory to anti-vascular endothelial growth factor (VEGF) therapy was referred to our hospital. Fundus examination showed retinal arterial MaAs and hard exudations.

Purpose Of Review: Diagnosing sarcoidosis remains challenging. Histology findings and a variable clinical presentation can mimic other infectious, malignant, and autoimmune diseases. This review synthesizes current evidence on histopathology, sampling techniques, imaging modalities, and biomarkers and explores how emerging 'omics' and artificial intelligence tools may sharpen diagnostic accuracy.

Sacroiliac joint dysfunction (SIJD) is a significant contributor to lower back pain, and the condition often mimics other lower back pain syndromes, necessitating accurate diagnosis through history, physical examination, provocative tests, and imaging studies. We present a 67-year-old man with a history of sarcoidosis, deep vein thrombosis, prostate cancer, and chronic low back pain, who experienced persistent right-sided back and hip pain despite multiple surgeries, including bilateral sacroiliac joint (SIJ) fusion. Conservative treatments provided only partial relief.

Background: Prior work has reported differences in the prevalence of sarcoidosis as well as patient outcomes based on race, gender, and socioeconomic status. We investigated whether sociodemographic factors were associated with referral to pulmonary medicine at a large academic center for patients with an incident international classification of diseases (ICD) diagnosis of sarcoidosis.
Methods: We conducted a single-center retrospective study examining the associations between sociodemographic factors and time to pulmonary medicine referral in patients with an incident ICD diagnosis of sarcoidosis between October 31, 2011, and October 30, 2021.

This retrospective observational study was conducted in a tertiary care hospital to assess the challenges involved in the diagnosis of cardiac sarcoidosis. The study included a total of 42 patients with primary cardiac manifestation of sarcoidosis and imaging evidence of patchy myocardial inflammation. Ventricular tachycardia(20) was the most common cardiac manifestation, followed by heart failure(12) and complete heart block(10).
