This case report describes a complex full-step asymmetrical Class II division 1 high-angle in an adult patient treated by extraction of compromised first molars with a preadjusted lingual appliance. Since the patient presented severe sagittal and vertical discrepancies combined with an Izard orthofrontal profile with upper lip protrusion, an extraction camouflage was performed with the twofold aim of obtaining ideal occlusal relationship and profile improvement, correcting occlusal plane cant by selective intrusion with interradicular miniscrews. Appropriate biomechanical strategies, including extraction choice and anchorage control during space closure, were needed to achieve the planned results. This case report demonstrates the possibility of solving successfully severe sagittal and vertical discrepancies with significant asymmetric component in adult patient without surgical treatment by means of a completely invisible technique, with the extraction of the most compromised teeth in both arches. This report also underlines the need for careful planning during both diagnostic and treatment phases, with appropriate skeletal anchorage management, in order to obtain the best results.

Birt-Hogg-Dubé syndrome (BHDS) is an autosomal dominant inherited disorder caused by mutation in a tumour suppressor gene, FLCN, leading to skin tumours (fibrofolliculomas), renal tumours and pulmonary cysts. Lung involvement is predominantly observed in 70% of the cases of BHDS, manifesting in the form of recurrent primary spontaneous pneumothorax. This video tutorial showcases the surgical management of recurrent right primary spontaneous pneumothorax in a young adult with a history of multiple episodes of bilateral pneumothorax managed by surgical intervention previously.

The patient had rheumatic heart disease, which resulted in severe aortic and mitral valve regurgitation. Repair of both valves was performed at 9 years of age. During surgery, the retracted aortic valve cusps required extension with bovine pericardial patches and suture reduction annuloplasty, and the mitral valve was repaired using a Cosgrove-Edwards (Edwards Lifesciences LLC, Irvine, CA) annuloplasty band.

Myoepithelial carcinoma (MECA) is a malignant neoplasm composed exclusively of myoepithelial cells and accounts for less than 1% of all salivary gland tumors. Its diagnosis is often challenging due to histologic overlaps with benign lesions and its variable morphologic presentation. Although molecular profiling has emerged as a valuable tool in salivary gland tumor classification, the genetic landscape of MECA remains incompletely defined.
