The SA node is protected from the overdrive suppression by the other tissues or tachyarrhythmias by means of atrio-sinus entrance block. A 57-year-old woman presented with symptomatic tachy-brady syndrome and severe LV dysfunction. Electrophysiological study revealed presence of left lateral accessory pathway. After ablation of the accessory pathway, tachycardia could no longer be induced. Her LV systolic function significantly improved on follow up and an external loop recorder showed no episodes of bradycardia. Therefore, we conclude that the bradycardia was due to overdrive suppression of SA node caused by AVRT, and the reversible cardiomyopathy was also due to high tachycardia burden.

Unlabelled: Ventricular fibrillation (VF) storm is a life-threatening condition that is particularly challenging to manage when resistant to conventional therapies such as antiarrhythmic drugs, deep sedation, overdrive pacing, and hemodynamic support. We report a case of a 55-year-old male with ischemic heart disease and recurrent VF storm unresponsive to initial treatments, requiring percutaneous cardiopulmonary support to stabilize hemodynamics. A left ventricular inferoseptal Purkinje-related premature ventricular contraction (PVC) was identified as the VF trigger, with continuous propagating activity involving Purkinje and myocardial substrates as the driver.

Cues such as fast-food advertisements associated with food can provoke food cravings which may lead to unhealthy overeating. To effectively control such cravings, we need to better understand the factors that reduce food cue reactivity and reveal corresponding 'anti-craving' brain mechanisms. We previously reported that access to environmental enrichment (EE) that provides cognitive and physical stimulation in mice reduced cue-evoked sucrose seeking and prelimbic cortex (PL) neuronal reactivity.
