Gallstone ileus results in a mechanical small bowel obstruction when an itinerant gallstone tumbles downstream and obstructs the bowel lumen. Associated proximal intestinal injury with perforation is rare, and concomitant perforation of multiple segments of bowel in the setting of gallstone ileus has never been reported in the literature. We are reporting the case of a 67-year-old female patient who had gallstone ileus causing perforation of multiple segments of small bowel. At operation, she was found to have a 3.2-cm gallstone lodged at the terminal ileum, perforation of both the mid ileum and mid jejunum, and gross enteric spillage. She underwent removal of the gallstone and small bowel resection × 2. She was initially left in discontinuity with an open abdomen. She returned to the operating room 2 days later for bowel anastomosis and abdominal closure. She recovered well after surgery and was discharged home.

As an uncommon etiology of bowel obstruction that predominantly affects the elderly, gallstone ileus warrants attention because it frequently arises from cholecystoenteric fistulas and carries disproportionate morbidity when recognition is delayed. This case report details an unusual presentation in an elderly male patient with no history of abdominal surgery who exhibited nonspecific abdominal symptoms. Imaging studies identified Rigler's triad, and computed tomography (CT) confirmed the presence of an ectopic gallstone in the proximal ileum.

Gallstone ileus, a rare complication of cholecystitis, occurs when the distal ileum becomes obstructed by gallstone causing, mechanical bowel obstruction. Gallstone ileus with history of cholecystectomy is exceptionally rare, with few cases documented in literature. This case report details the discovery of a large, impacted gallstone 16 years postcholecystectomy.

Introduction: Gall stone ileus is a rare complication of cholelithiasis which typically presents with obstruction in the small intestine. However, it can rarely mimic a mass when it presents in unusual sites like the sigmoid colon as in our case.
Case Presentation: We present a 42-year-old woman with a history of bariatric surgery, diverticulitis status post sigmoid colectomy and decompensated cirrhosis complicated by hepatic encephalopathy who presented to the hospital with concern for altered mental status and was diagnosed with grade III hepatic encephalopathy due to lactulose non adherence.

This study assessed the diagnostic value of CT imaging and its correlation with histopathological findings in confirmed cases.
