Severe, untreated tricuspid regurgitation is associated with worse clinical outcomes. While isolated tricuspid valve (TV) surgery has been linked to poor long-term outcomes, transcatheter TV therapies, including edge-to-edge repair and transcatheter tricuspid valve replacement (TTVR), have emerged as effective alternatives and have been shown to improve outcomes, leading to their regulatory approval in the United States. Conduction system abnormalities are commonly seen among patients undergoing TTVR due to the close proximity of the atrioventricular node and the His bundle to the TV annulus.

Background: A subcutaneous implantable cardioverter defibrillator (S-ICD) is an alternative to a conventional transvenous implantable cardioverter defibrillator for preventing sudden cardiac death. Although posterior chest S-ICD implantation has been recommended for better defibrillation outcomes, little is known about the optimal S-ICD positioning for R-wave detection. Herein, we report two cases of S-ICD recipients in whom antero-inferior chest positioning improved R-wave detection after posterior chest positioning failed.

Background: Older pacemaker and defibrillator leads are established risk factors for complications during transvenous lead extraction (TLE); however, its impact on procedural outcomes remains clinically relevant.
Objective: This study aimed to compare TLE outcomes between patients with cardiac leads ≥10 years old (Group A, n = 858) and those with leads <10 years old (Group B, n = 1666).
Methods: This retrospective comparative analysis focused on baseline and lead characteristics, procedural outcomes, and identifying predictors of adverse events.
