Article Abstract

SonoVue (Bracco, Milan, Italy) is a drug used in ultrasonography for the purpose of increasing the echogenicity of blood or fluids by improving the signal-to-noise ratio. : We described a case of anaphylaxis due to SonoVue and performed a literature review. : We reported a case of anaphylaxis secondary to the administration of SonoVue and described all the 13 literature cases. Given its widespread use and the potentially dangerous nature of the reactions it can cause, it is advisable to know how to promptly recognize fatal reactions.

A male patient in his early 60s was referred to the cardiology department for evaluation of a persistent apical ventricular thrombus following a myocardial infarction. Transthoracic echocardiography could not rule out the presence of an apical thrombus, leading to the intravenous administration of the contrast agent sulphur hexafluoride (SonoVue). The patient quickly exhibited signs of anaphylaxis accompanied by haemodynamic shock, resulting in cardiac arrest.

Echo contrast agents, such as SonoVue, (Bracco, Milan, Italy) are often used to enhance diagnosis. Although their use is safe, some rare side effects could be severe or even fatal. We are reporting a case of severe systemic allergic reaction after infusion of SonoVue.

Initially, a set of guidelines for the use of ultrasound contrast agents was published in 2004 dealing only with liver applications. A second edition of the guidelines in 2008 reflected changes in the available contrast agents and updated the guidelines for the liver, as well as implementing some non-liver applications. Time has moved on, and the need for international guidelines on the use of CEUS in the liver has become apparent.
