IntroductionWe report the successful use of erector spinae (ESP) plane block in the management of a patient with severe respiratory failure secondary to chest trauma requiring invasive ventilation and Veno-venous extracorporeal membrane oxygenation (V-V ECMO).Case reportA 64-year-old man with flail chest and severe respiratory failure required V-V ECMO. An ESP plane block on day 3 enabled extubation, mobilisation, and secretion clearance, leading to ECMO weaning after six days and discharge 18 days post-injury.

Objective: To provide a comprehensive summary of the technological evolution, clinical protocols, mechanisms of action, and current research progress of tibial transverse transport (TTT), with the goal of facilitating its standardized application in clinical practice.
Methods: A systematic review of both domestic and international literature on TTT for the treatment of diabetic foot was conducted. The analysis encompassed technical developments, surgical protocols, combination therapies, regenerative mechanisms, and clinical outcomes.

Background: Wound healing is challenging in cases of impaired microcirculation, leading to wound chronicity, decreased quality of life, and increased morbidity. Surgical site infections (SSIs) pose a significant challenge in diabetic, hypertensive, and obese patients due to impaired microcirculation. Negative pressure wound therapy (NPWT) is a widely used adjunct in wound management, but its optimal parameters in this subgroup remain uncertain.

Inorganic nitrate plays a crucial role in the regulation of cerebral blood flow and neurotransmission through its conversion to nitric oxide (NO). Astrocytes are star-shaped glial cells and contribute to maintain the blood-brain barrier integrity, regulate neuronal metabolism, support synaptic plasticity and facilitate neurovascular coupling. Inorganic nitrate widely distributed through all organs, with main reservoirs in skeletomuscular and skin tissues.

Background: Ischaemia is a critical complication, and can result in poor surgical outcomes. While intra-operative overt ischaemia can be perceived with the naked eye, timely recognition of borderline perfusion can prevent post-operative ischaemic complications, which is particularly relevant for colorectal anastomoses. Consequently, there is a clinical need for new technologies to intra-operatively assess tissue oxygenation (indicative of end organ perfusion), with minimal disruption to the surgical workflow.
