Prior research has raised concerns regarding the use of macrolides and their association with an increased risk of cardiovascular events. We conducted a cohort study, where we explored the cardiovascular risks associated with the treatment of COPD patients using macrolide antibiotics-namely azithromycin, clarithromycin, and roxithromycin-with amoxicillin serving as a reference. The study focused on COPD patients in an outpatient setting and included a thorough 3-year follow-up. Patients were categorized into four groups based on their treatment. The primary analysis utilized an adjusted Cox model, supplemented by sensitivity analysis through inverse probability of treatment weighting. No significant differences were found in major adverse cardiovascular events (MACE-stroke, acute myocardial infarction, cardiovascular death) between the macrolide groups, and the amoxicillin/hazard ratios (HR) were azithromycin HR = 1.01, clarithromycin HR = 0.99, and roxithromycin HR = 1.02. Similarly, sensitivity analysis showed no disparities in all-cause mortality and cardiovascular death among the groups. Overall, the study revealed no evidence of increased risk of MACE, all-cause mortality, or cardiovascular death in COPD patients treated with these macrolides compared to amoxicillin over a 3-year period.

Importance: Transthyretin cardiac amyloidosis (ATTR-CA) is an underdiagnosed but treatable cause of heart failure (HF) in older individuals that occurs in the context of normal wild-type (ATTRwt-CA) or an abnormal inherited (ATTRv-CA) TTR gene variant. While the most common inherited TTR variant, V142I, occurs in 3% to 4% of self-identified Black Americans and is associated with excess morbidity and mortality, the prevalence of ATTR-CA in this at-risk population is unknown.
Objective: To define the prevalence of ATTR-CA and proportions attributable to ATTRwt-CA or ATTRv-CA among older Black and Caribbean Hispanic individuals with HF.

Purpose Of Review: Despite major advances in the treatment and prevention of atherosclerotic cardiovascular disease (ASCVD), a substantial burden of residual risk remains Obesity has been redefined as a primary and independent drivers of cardiovascular morbidity and mortality warranting focused attention.
Recent Findings: Obesity is now recognized as a chronic disease and a central contributor to residual cardiovascular risk through mechanisms including systemic inflammation, insulin resistance, dyslipidemia, and endothelial dysfunction. This review addresses the limitations of conventional obesity management and highlights emerging pharmacological therapies targeting the underlying adiposopathy.

Here, we aimed to identify potential anti-atherosclerosis drugs by an integrated approach combining network medicine-based prediction with empirical validation. Among the top drugs predicted by the preferred algorithm, mesalazine─a drug traditionally used to treat inflammatory bowel disease, was selected for in vivo validation in ApoE mouse model of atherosclerosis.
