Pool palms and pool toes are friction or pressure-induced aquatic dermatoses resulting from extensive and repetitive rubbing of fingers, palms, soles, and toes against the rough anti-slippery surfaces or edges of pools. Aquagenic syringeal acrokeratoderma is a sporadic, episodic, and acquired disorder of palmar skin linked to water exposure. Herein we describe a case of aquatic/aquagenic dermatosis that presented clinical and diagnostic difficulties.

Aquagenic syringeal keratoderma (ASK), rare in males, is characterized by the rapid onset of edematous palmar wrinkling with small white papules after brief contact with water or sweat. A 24-year-old atopic male presented with a 2-week subacute history of bilateral palmar edema with whitish-colored papules after exposure to water, 3 months after having had COVID-19 infection treated with a full course of ritonavir-boosted nirmatrelvir (PAXLOVID). He had received 3 COVID-19 vaccines (Pfizer, New York, NY) about 12 months prior.

Background: Aquagenic wrinkling of the palms (AWP) is an excessive and early palmar wrinkling occurring after brief immersion to water (BIW), and has been reported as a frequent finding among Cystic Fibrosis (CF) patients.

Objectives: To investigate any associations of CF patients presenting AWP with other disease characteristics and explore the pathomechanism of AWP phenomenon.

Methods: We evaluated AWP in CF patients and assessed the AWP parameters of palmar wrinkling, oedema, papules, pruritus and pain at 3, 7 and 11 min after a BIW test with other disease characteristics.
