In this work, the case of a 70-year-old Caucasian woman affected by cryptogenic stroke is reported. After discarding other sources of embolism, a transesophageal echocardiogram was performed, which revealed the presence of a double interatrial septum associated with a left-sided atrial pouch. The persistent interatrial space was identified as the most probable source of thrombus.

Objectives: Balloon atrial septostomy (BAS) is a palliative procedure mainly for newborns with transposition of the great arteries, currently, this procedure has a class IA recommendation. However, in some cases, this procedure does not achieve an adequate interatrial defect, due mainly to a thickened interatrial septum, this situation led to the development of various techniques to tackle this problem. Therefore, we made a modification to the original technique using two balloons simultaneously instead of one.

During percutaneous left atrial appendage closure using the Watchman device, the access sheath usually remains in the left atrium. We experienced a case in which we could not recapture the device, because the access sheath was accidentally pulled back to the right atrium. Because bailout from the access sheath was unsuccessful, balloon dilation of the interatrial septum was performed using a double-sheath technique via the contralateral femoral vein.

The phenomenon of double interatrial septum (DIAS) represents a particularly rare subtype of atrial septal malformation, characterized by the presence of dual membranes separating the atria, resulting in a distinctive interatrial space. This unique anatomical structure has been linked to a paradoxical right-to-left shunt, potentially contributing to embolic ischemic strokes. Within this context, we report a rare case of a 34-year-old female who presented with a transient ischemic attack (TIA) and was diagnosed with patent foramen ovale (PFO) and a small adjacent atrial septal defect (ASD), along with the presence of a DIAS.

The atrial flow regulator is a new self-expandable double-disc fenestrated device providing a calibrated inter-atrial communication. Paediatric reports are scarce. We herein describe a case of complete atrioventricular block complicating the exemption use of an atrial flow regulator in a 5-kg infant with transposition of the great arteries, ventricular septal defect, and right ventricular outflow tract obstruction.
