PurposeCritical Care Echocardiography (CCE) is now a major tool in assessments of ICU patients. Objective: To describe the effects of dexmedetomidine on sedation, pain, respiratory status, and hemodynamics in neonates.
Methods: A retrospective study of 206 infants in a level IV NICU (2016-2021) receiving continuous dexmedetomidine infusion. Pain/sedation scores, BP, vasopressor and inotrope score (VIS), and concomitant sedatives/analgesics (CSA) were recorded before and every 3-4 h for 24 h.

Introduction: Amlodipine, a dihydropyridine calcium channel blocker, is a growing cause of poisoning fatalities in the United States. As it takes several hours following ingestion for amlodipine peak plasma concentrations to be reached, the severity of poisoning may not be immediately apparent to clinicians. This study aimed to stratify reported amlodipine exposure doses with the severity of clinical outcomes.

Background: Vasopressors and inotropes remain the cornerstone in treatment of acute myocardial infarction-related cardiogenic shock (AMI-CS). Milrinone and dobutamine are both commonly used, yet the optimal inotrope remains unclear. We aimed to identify factors associated with milrinone and dobutamine treatment and assess their effects on 30-day mortality in a large real-world cohort of AMI-CS patients.

Adrenal crisis, a life-threatening complication of adrenal insufficiency, can present perioperatively with refractory hypotension and electrolyte imbalances. This case report describes a 77-year-old woman with multiple comorbidities who developed intraoperative shock unresponsive to high-dose vasopressors during aortobifemoral bypass surgery. Persistent hypokalemia and hypoglycemia raised suspicion of adrenal insufficiency, prompting empiric intravenous (IV) methylprednisolone (75 mg), which rapidly resolved hypotension and obviated further inotropic support.
