Background: People experiencing homelessness (PEH) have worse health than the general population, and higher rates of hospitalization. The transition period after discharge from hospital is often challenging for PEH, in part due to loss to follow-up, competing priorities, housing instability, and the absence of a primary care provider. In-patient hospital stays represent a window of opportunity to intervene and connect with patients, supporting them to stay in hospital and complete their treatment plan, identify and address their social needs, and support their transition of care into the community.

The Venous Excess Ultrasound Score (VExUS) has produced great interest in venous Doppler ultrasound as a noninvasive means to evaluate right heart hemodynamics. While this score includes Doppler morphologies from sub-diaphragmatic veins, the physiology of transcutaneous venous Doppler velocimetry and its change with cardiac pathology was first studied and described in the internal jugular vein (IJV). Over 50 years ago, the systolic and diastolic velocity waves of the IJV were found to describe the x'- and y-descents of the jugular venous pulse (JVP) in sickness and in health.

Background: Prior practice in a 523-bed community hospital required automated dispensing cabinets (ADCs) to be replenished daily by early afternoon to ensure accurate drug ordering. Because of the tight time window, technicians frequently refilled ADCs during peak medication administration times. After a positive report from a smaller facility in the health system, the study facility implemented a quality improvement project to optimize the ADCs.

Background: Venous thromboembolism (VTE) is a significant complication following bariatric surgery, with most events occurring after discharge. While inpatient VTE chemoprophylaxis is well-established, its extension post-discharge remains controversial. Apixaban, an oral direct factor Xa inhibitor, is a potential alternative for this purpose, offering convenience and a favorable safety profile compared to enoxaparin.
