Introduction: Pulmonary embolism (PE) is a life-threatening condition with well-defined management strategies; however, the presence of a clot-in-transit (CIT)-a mobile thrombus within the right heart-introduces a uniquely high-risk scenario associated with a significantly elevated mortality rate. While several therapeutic approaches are available-including anticoagulation, systemic thrombolysis, surgical embolectomy, and catheter-directed therapies-there is no established consensus on a superior treatment modality. Catheter-based mechanical thrombectomy has emerged as a promising, minimally invasive alternative that mitigates the bleeding risks of systemic thrombolysis and the invasiveness of surgery.

Congenital diaphragmatic hernia (CDH) is a serious congenital anomaly often associated with pulmonary hypoplasia and persistent pulmonary hypertension of the newborn (PPHN). Central vascular access such as umbilical arterial catheters (UACs) is routinely used in neonatal intensive care but is associated with the risk of vascular complications, including thromboembolic events. We present a case of preterm dichorionic diamniotic (DCDA) twins born at 34 weeks of gestation with antenatally diagnosed CDH.

Background: Venous thromboembolism (VTE) is a considerable source of morbidity, mortality, and economic burden within orthopaedic surgery. Our study aimed to analyze the characteristics and reasons for lawsuits pertaining to VTE levied against orthopaedic surgeons.

Methods: The Westlaw database was queried for cases filed between 1980 and 2023 against orthopaedic surgeons involving VTE, using the search terms "orthopaedic", "blood clot," "deep vein thrombosis," "venous thromboembolism," and "pulmonary embolism.
