Conversion of patellofemoral arthroplasty to total knee arthroplasty (TKA) has been described as similar to primary TKA, although it may come with more challenges and worse outcomes. With the increased rate of revision following conversion TKA vs primary TKA, robotically assisted TKA provides an alternative technique to manual conversion. We present 3 cases of robot-assisted conversion of prior patellofemoral arthroplasty to TKA with good mechanical and clinical outcomes and no intraoperative complications.

Patellofemoral arthroplasty (PFA) is useful and effective option for treating patients with isolated patellofemoral osteoarthritis. The concept of functional positioning (FP) in PFA focuses on resurfacing the trochlea and restoring normal patellar tracking, while keeping the joint anatomy and kinematics. Even though the patellar liner cannot yet be placed with robotic assistance, robotic tools still help surgeons manage and optimize patellar tracking during surgery.

Patellar instability remains one of the most challenging and persistent complications following total knee arthroplasty (TKA), significantly affecting patient satisfaction and functional outcomes. Despite advances in implant technology and surgical techniques, patellofemoral maltracking remains a leading cause of anterior knee pain and one of the most frequent non-infectious reasons for revision surgery. The etiology is complex and often involves a combination of component malposition, soft-tissue imbalance, patient-specific anatomical risk factors, and suboptimal implant design.

Introduction: Patellar dislocation following total knee arthroplasty (TKA) is an uncommon but functionally significant complication. Its management requires careful evaluation of prosthetic alignment, patellofemoral mechanics, and soft tissue integrity. The aim of this study was to describe a surgical stabilization technique using a medial retinacular flap fixed to the patella with suture anchors, combined with controlled lateral retinacular release, and to analyze its clinical and functional outcomes.

Background And Purpose: Patellofemoral arthroplasty (PFA) is a rare surgical procedure for isolated patellofemoral osteoarthritis (PFOA). This study compares patient demographics, long-term survival rates, revision risks, and causes of revision in PFA with total knee arthroplasty (TKA) and unicompartmental knee arthroplasty (UKA).
Methods: Data from the Norwegian Arthroplasty Register (NAR) (1994-2022) included 725 PFA, 102,135 TKA, and 14,315 UKA procedures.
