Background: The presence of visual imagery in dreams of congenitally blind people has long been a matter of substantial controversy. We set to systematically review body of published work on the presence and nature of oneiric visuo-spatial impressions in congenitally and early blind subjects across different areas of research, from experimental psychology, functional neuroimaging, sensory substitution, and sleep research.
Methods: Relevant studies were identified using the following databases: EMBASE, MEDLINE and PsychINFO.
Results: Studies using diverse imaging techniques and sensory substitution devices broadly suggest that the "blind" occipital cortex may be able to integrate non-visual sensory inputs, and thus possibly also generate visuo-spatial impressions. Visual impressions have also been reported by blind subjects who had near-death or out-of-body experiences.
Conclusion: Deciphering the mechanistic nature of these visual impression could open new possibility in utilization of neuroplasticity and its potential role for treatment of neurodisability.

With positron emission tomography followed by functional magnetic resonance imaging (fMRI), we demonstrated that rapid eye movements (REMs) in sleep are saccades that scan dream imagery. The brain "sees" essentially the same way while awake and while dreaming in REM sleep. As expected, an event-related fMRI study (events = REMs) showed activation time-locked to REMs in sleep ("REM-locked" activation) in the oculomotor circuit that controls saccadic eye movements and visual attention.

During his lessons, Jean-Martin Charcot (1825-1893) explored the role of dreams in neurological and psychiatric conditions. While often overshadowed by Sigmund Freud's later contributions, Charcot's observations provide valuable insights into the intersection of neurology and dream analysis. Charcot identified characteristic dream patterns linked to specific conditions: aggressive animal dreams in hysteria, recurrent traumatic nightmares in traumatic neurosis, auditory hallucinations in chronic alcoholism, and enhanced motor imagery in phantom limb syndrome.

Both voluntary mental imagery and dream imagery involve multisensory representations without externally present stimuli that can be categorized as offline perceptions. Due to common mechanisms, correlations between multisensory dream imagery profiles and multisensory voluntary mental imagery profiles were hypothesized. In a sample of 226 participants, correlations within the respective state of consciousness were significantly bigger than across, favouring two distinct networks.
