Brucellosis is a neglected zoonotic infection in Nepal that is often underdiagnosed, particularly in regions considered to have low prevalence. Its presentation can mimic autoimmune or hepatic disorders, complicating timely diagnosis and management. Coexistence with hereditary conditions such as Gilbert syndrome can further obscure the clinical picture.

Surgical clip migration to the common bile duct is a rare late complication, typically originating from clips placed at the cystic duct and most commonly reported after laparoscopic cholecystectomy. We present an exceptionally rare case of obstructive jaundice caused by clip migration from the liver dissection plane, rather than from the cystic duct, occurring 12 years after laparoscopic liver resection (LLR) and cholecystectomy and associated with chronic biliary inflammation. A 73-year-old man underwent LLR of segments 4a + 5 and cholecystectomy for hepatocellular carcinoma and was discharged on postoperative day 12 without any complications.

Sickle cell hepatopathy (SCH) is an umbrella term relating to liver disease in sickle cell disease (SCD). This term ranges from common etiologies such as cholelithiasis to disease-specific causes such as sickle cell intrahepatic cholestasis (SCIC), a rare but significant complication of SCD capable of progressing to liver failure and consideration of transplantation. We report the case of a 24-year-old male with SCD who presented with jaundice, encephalopathy, uncontrollable epistaxis, and pseudohematemesis and was found to have hyperbilirubinemia, coagulopathy, portal hypertension, and acute kidney injury (AKI).

Background/aims: This study aimed to compare the stent patency between the novel multi-hole self-expandable metallic stent (MH-SEMS) and conventional partially covered SEMS (PC-SEMS) for malignant distal biliary obstruction (MDBO) in patients with pancreatic carcinoma.

Methods: This retrospective study compared stent patency between patients with MH-SEMS (n=43) and those with PC-SEMS (n=94). Secondary outcomes were overall survival (OS), incidence of recurrent biliary obstruction (RBO), causes of RBO, and adverse events (AEs).

A 74-year-old woman underwent endoscopic retrograde cholangiopancreatography because of obstructive jaundice, caused by pancreatic cancer. The patient received a plastic biliary stent (half-pigtail type) placement into the left hepatic bile duct for biliary drainage. After 28 days, the patient presented to our hospital with fever and jaundice.
