Perceptions of errors associated with healthcare information technology (HIT) often depend on the context and position of the viewer. HIT vendors posit very different causes of errors than clinicians, implementation teams, or IT staff. Even within the same hospital, members of departments and services often implicate other departments. Organizations may attribute errors to external care partners that refer patients, such as nursing homes or outside clinics. Also, the various clinical roles within an organization (e.g., physicians, nurses, pharmacists) can conceptualize errors and their root causes differently. Overarching all these perceptual factors, the definitions, mechanisms, and incidence of HIT-related errors are remarkably conflictual. There is neither a universal standard for defining or counting these errors. This paper attempts to enumerate and clarify the issues related to differential perceptions of medical errors associated with HIT. It then suggests solutions.

Visuospatial perception, which is based on the comprehension of objects and space, requires spatial attention to the surrounding environment. Stimulus-related elements that affect visuospatial tasks include object geometry, familiarity, complexity, and picture plane versus depth rotation. The dorsal stream pathway from the visual cortex, which is implicated in spatial processing, reflects the spatial component needed to orient the focus of attention to the location of the expected target stimulus.

Primary open-angle glaucoma (POAG) is characterized by chronic progressive damage to the retinal ganglion cell layer (GCL) and their axons, leading to gradual visual function loss. Currently, the gold standards for structural and functional assessment of the retina in glaucoma are static automated perimetry (SAP) and optical coherence tomography (OCT). However, in clinical practice, data from SAP and OCT may be insufficient to reliably determine the stage of glaucomatous optic neuropathy, monitor its progression, or differentiate it from other causes of visual dysfunction.

The neural circuits that transmit the sense of pain and how pain is encoded by these circuits are still poorly understood.Mechanical allodynia is a prominent form of chronic pain characterized by painful responses to innocuous touch that develops as a consequence of nerve damage and inflammation. Here, we show that alterations to the normal log-normal distribution of neuronal activity and structure of neural correlations between neurons in the dorsal column nuclei (DCN) constitute a signature feature of mechanical allodynia, with the transmission of "allodynic" light touch information to the thalamus by somatostatin-positive projection neurons in the DCN being essential for its expression and development.
