When interrogating a pacemaker, there are multiple elements that can be measured to provide insight into a pacemaker's integrity and function. Changes in any of those elements outside the normal range may indicate complications such as lead malfunction, lead detachment, or even cardiovascular structural changes. Background: Radiation therapy (RT) can cause cardiac implantable electronic device (CIED) malfunction, primarily reset. Given changes in RT and CIED technologies, large observational studies examining malfunction of contemporary CIEDs during modern-day RT are needed to guide clinical practice.

Methods: Electronic medical records of all consecutive patients with CIEDs who underwent RT at a large tertiary cancer center between January 2015 and January 2022 were reviewed.

Introduction: CIED patients are routinely advised against physical activity with a risk of collision because of expected damage to the implanted device. However, no data support this practice.

Methods: Sixteen CIED systems (6 pacemakers, 6 ICD, 4 CRT-D) from all manufacturers were implanted subcutaneously in a porcine thorax and increasing weights were dropped on the experimental setting.

A systematic and multidisciplinary approach is needed to manage cancer patients with a cardiac implantable electronic device undergoing radiation therapy. We report a statement from the Italian Association of Hospital Cardiologists (ANMCO) to provide guidance for clinicians involved in the care continuum of this challenging clinical setting. A comprehensive careful assessment of radiotherapy type, treatment plan and cardiac issues is recommended in order to minimize the risk of device malfunction.
