A woman in her 40s presented with malaise, nausea, reduced appetite, abdominal distention, loose stools and weight loss. Symptoms had started 6 months earlier and worsened in the last 2 weeks. CT enterography showed hypotonic dilated small bowel loops in absence of any mechanical obstruction. Endoscopic examinations including capsule endoscopy did not reveal any obstructing lesion, but a delayed small bowel transit time of the capsule. Duodenal histology revealed Marsh 3a villous atrophy. Secondary causes of intestinal pseudo-obstruction and villous atrophy were investigated. trophozoites were found in the stools and in the duodenal biopsies. The patient's symptoms quickly resolved after metronidazole treatment with complete normalisation of duodenal histology.

Background: Idiopathic mesenteric phlebosclerosis (IMP) is a rare condition that causes ischaemic colitis, has various clinical manifestations, and may even be asymptomatic, often resulting in clinical misdiagnosis.
Case Summary: We report the case of a 44-year-old woman with chief complaints of abdominal pain, vomiting and decreased defecation. One year prior, the patient was asymptomatic and therefore misdiagnosed with inflammatory bowel disease based on incidental findings on an abdominal computed tomography (CT) scan.

Mutations in the filamin A (FLNA) gene cause a broad range of disorders, affecting musculoskeletal, nervous, vascular, and gastrointestinal systems, collectively known as filaminopathies. In contrast to previously described mutations in the long isoform of , which alter the reading frame and lead to loss of Filamin A expression resulting in congenital short bowel syndrome or chronic intestinal pseudo-obstruction in pediatric patients, here we present the clinical and genetic features of an adult patient with chronic intestinal pseudo-obstruction in whom whole exome sequencing revealed a novel missense mutation (p.Gly19Val) in gene.
