An immunocompetent 49-year-old man presented with swelling and pain in the lower region of his left leg that had lasted for 4 weeks. The diagnosis was severe pyomyositis and osteomyelitis in the lower left leg caused by hypervirulent (hvKP) along with multiple metastatic infections in the kidneys, lungs, and brain originating from an anorectal abscess. A virulence-gene analysis revealed that the isolated harbored , , , , , , and -virulence genes and belonged to the K1 capsular serotype. After repeated abscess drainage procedures, intravenous ceftriaxone was administered for more than 10 weeks, and the patient's infection was controlled. We focused on the clinical features of hvKP originating from an anorectal abscess without a pyogenic liver abscess. We suggest that hvKP be considered a causative pathogen of pyomyositis and osteomyelitis resulting in multiple metastatic infections in an immunocompetent patient, and more information on the unexpected multiple metastatic infections should be obtained from a virulence analysis of .

Background: Primary Anorectal malignant melanoma (pARMM) is an exceedingly rare and aggressive malignancy, accounting for approximately 1% of anorectal cancers. It originates from melanocytes in the anorectal mucosa and lacks distinctive clinical features, leading to frequent misdiagnosis and advanced presentation.
Methods: A retrospective analysis was conducted on 9 patients (1 male, 8 females; median age 59 years) with histopathologically and immunohistochemically confirmed ARMM who underwent surgical resection (Wide Local Excision, WLE = 4; Abdominoperineal Resection, APR = 5) and had complete follow-up data (median 19 months, up to May 2025).

Neisseria meningitidis (Nm) is responsible for invasive meningococcal disease (IMD), with relatively few reports of causing urogenital and anorectal infections. This study reported the emergence of three urethritis cases in male adults caused by Nm isolates during 2021 and 2024 in Shanghai, China. Phylogenetic analysis revealed that the three isolates were closely related with strains causing IMD in the US (Y:ST-1466) and Japan (NG:ST-11026), respectively.
