Cerebral cavernous malformations (CCMs) are known to be angiographically occult malformations with low perfusion of blood flow. Near-infrared indocyanine green (ICG) video angiography allows for intraoperative observation and documentation of blood flow in large and small vessels. Developmental venous anomalies (DVAs) are thought to be the most common cerebral vascular abnormality. The opportunity to differentiate intraoperatively between normal veins and DVA draining veins might be useful in the event of a possible venous sacrifice. Coagulation of the DVA can lead to devastating consequences. ICG reliably demonstrates margins between CCM and the venous structures. Purpose: Isolated spinal aneurysms (iSAs) are rare, with an uncertain natural history and no established treatment guidelines. Multiple iSAs are even more uncommon, complicating treatment decisions.
Methods: This study reports a case of a ruptured radiculo-pial artery aneurysm in a patient with multiple iSAs, treated with surgical excision, assisted by intraoperative neurophysiological monitoring (IONM).

Kommerell's diverticulum (KD) combined with a right-sided aortic arch (RAA) and an aberrant left subclavian artery (ALSA) is a rare congenital vascular anomaly causing significant compressive dysphagia. Treatment options, including open surgery, thoracic endovascular aortic repair and hybrid approaches, are debated due to anatomical complexities. We report a 48-year-old female with dysphagia from symptomatic KD, RAA and ALSA, clearly delineated by preoperative computed tomography angiography.

The brain arteriovenous malformation (BAVM) within language-eloquent area poses a significant surgical challenge, demanding meticulous planning to ensure both preservation of language function and curative resection. This report details the successful microsurgical resection of a Spetzler-Martin grade II BAVM located in Wernicke's area in a 51-year-old male, ruptured three weeks ago and characterized by mild anomia. Following thorough discussion, the patient elected for microsurgery, providing informed consent, and the procedure was approved by the ethics committee.
