Microsecretory adenocarcinoma (MSA) of the salivary glands is a recently described entity. Due to lack of reported metastases, in 30 cases described until now, the designation as low-grade cancer was so far solely based on demonstration of local tumor invasion and in a single case with perineural invasion. We herein describe the first documented case with local recurrence and hematogenous metastases.

Microsecretory adenocarcinoma is a recently recognized low-grade salivary gland malignancy characterized by distinct histomorphological features, a specific immunophenotype, and recurrent gene fusion. This report describes 43-year-old woman with a two-decade history of a right buccal mucosa mass that exhibited recent enlargement, causing functional impairment. Surgical excision revealed a well-demarcated tumor with microcystic and tubular architecture, basophilic luminal secretions, and low proliferative activity (<1 mitotic figure/2mm).

Background: Microsecretory adenocarcinoma (MSA) is a newly identified entity in the WHO classification of salivary gland tumors characterized by MEF2C::SS18 fusion. It was previously considered as adenocarcinoma not otherwise specified (NOS). With the discovery of new gene fusions specifying distinct salivary gland tumors and restricting the diagnosis of adenocarcinoma NOS, five cases of MSA were recognized for the first time using targeted RNA sequencing.

To investigate the clinicopathological, immunohistochemical, and molecular genetic characteristics of microsecretory adenocarcinoma (MSA) of the salivary gland, and to improve the understanding of this rare tumor. Cases originally diagnosed as MSA at the Department of Oral Pathology, the Ninth People's Hospital of Shanghai Jiao Tong University School of Medicine were retrospectively collected. The cases of polymorphous adenocarcinoma and adenocarcinoma, not otherwise specified from January 2000 to January 2020 were reviewed to identify potential misdiagnosed MSA cases.

With the advancement of molecular testing and the routine use of immunohistochemical stains, salivary gland tumours previously categorized as adenoma or adenocarcinoma, not otherwise specified, are being reclassified with distinct diagnoses. Newly recognized benign entities include: sclerosing polycystic adenoma, keratocystoma, intercalated duct hyperplasia and adenoma, and striated duct adenoma. Newly recognized malignant salivary gland tumours include: microsecretory adenocarcinoma, sclerosing microcytic adenocarcinoma, and mucinous adenocarcinoma.
