Background: External iliac artery endofibrosis (EIAE) classically presents in cyclists with intimal thickening of the affected arteries. We investigated possible anatomical predisposing factors including psoas muscle hypertrophy, arterial tortuosity, inguinal ligament compression, and arterial kinking via a case-control comparison of symptomatic and contralateral limbs.
Methods: All patients with unilateral EIAE treated surgically at our institution were reviewed. Each patient's symptomatic side was compared with their contralateral side using paired t-tests. Psoas hypertrophy was quantified by the transverse cross-sectional area (CSA) at L4, L5, and S1 vertebral levels, and inguinal ligament compression was measured as the anterior-posterior distance between the inguinal ligament and underlying bone. Tortuosity index for diseased segments and arterial kinking were measured on TeraRecon.
Results: Of 33 patients operated on for EIAE from 2004 to 2021, 27 with available imaging presented with unilateral disease, more commonly left-sided (63%). Most (96%) had external iliac involvement and 26% had ≥2 segments affected: 19% common iliac artery, 15% common femoral artery. The symptomatic limb had greater mean L5 psoas CSA (1,450 mm vs. 1,396 mm, mean difference 54 mm, P = 0.039). There were no significant differences in L4 or S1 psoas hypertrophy, tortuosity, inguinal ligament compression, or arterial kinking. 63% underwent patch angioplasty, and 85% underwent additional inguinal ligament release. 84% reported postoperative satisfaction, which was associated with a greater difference in psoas hypertrophy at L4 (P = 0.022).
Conclusions: Psoas muscle hypertrophy is most pronounced at L5 and is associated with symptomatic EIAE. Preferential hypertrophy of the affected side correlates with improved outcomes, suggesting psoas muscle hypertrophy as a marker of disease severity.

For lumbar spinal canal stenosis, endoscopic spine surgery typically employs a unilateral approach. While this approach has the advantage of early access to the lamina, it risks damage to the facet joint on the entry side. Additionally, decompression of the ipsilateral lateral recess can be challenging, sometimes resulting in inadequate decompression laterally, leading to incomplete symptom relief.

We report the case of a 47-year-old woman who presented with left inguinal swelling; the biopsy of which showed high-grade serous adenocarcinoma. 68Ga-FAPI PET/CT revealed a tracer-avid lesion in the left adnexal region and an enlarged left inguinal nodal mass (site of biopsy). Multiple focal lesions were also seen at the hepatic dome, along the falciform ligament and at the right lateral abdominal wall, suspicious for peritoneal/metastatic deposits.

A 4-year-old, 0.81 kg, female intact, American guinea pig (Cavia porcellus) was presented for a one-day history of perianal swelling. Physical examination revealed a body condition score of 2 out of 9, a distended abdomen that was painful on palpation, an abnormal vulvar conformation that was enlarged, edematous, ulcerated, and erythematous with adhered foreign material, and a 1 × 1 cm firm mass of the right mammary gland.

The knowledge of anatomical variation of lumbar plexus helps to interpret the causes of lumbar plexopathies and at the same time minimise iatrogenic nerve injuries. This report describes a unique variation in the branching pattern of lumbar plexus in a 63-year-old male's cadaver. On both sides, the genitofemoral nerve emerged from the lateral border of the psoas major instead of from its ventral surface.

Cosmetic liquid silicone injections can result in silicone-induced inflammation, chronic mastitis, migration to distant locations and lymphadenitis which can result in false positive nodal and distant metastases. This study demonstrates a patient with multifocal FDG-avid right breast malignancy with FDG-avid bilateral axillary, mediastinal and abdominopelvic lymph nodes. She also had extensive FDG-avid foci in the bilateral breasts and subcutaneous gluteal regions secondary to cosmetic silicone administration.
