Objectives: This study aimed to determine the stool specimen collection and Clostridioides difficile (C. difficile) testing frequency from inpatients and long-term care facility (LTCF) residents with new-onset diarrhea.
Methods: A cross-sectional study was conducted in all wards of 9 adult hospitals (3532 beds) and 14 LTCFs (1205 beds) in Louisville, Kentucky to identify new-onset diarrhea (≥3 loose stools in the past 24 h and not present in the preceding 24 h) among Louisville adults via electronic medical record review, nurse interviews, and patient interviews during a 1-2 week observation period in 2018-2019.
Results: Among Louisville-resident inpatients, 167 patients with 9731 inpatient-days had new-onset diarrhea (1.7/100 inpatient-days). Stool specimens were collected from 32% (53/167); 12 (23%) specimens were laboratory-confirmed for C. difficile infection (CDI) (12.3 cases/10,000 inpatient-days). Among LTCF residents, 63 with 10,402 LTCF resident-days had new-onset diarrhea (0.6/100 LTCF resident-days). Stool specimens were collected from 32% (20/63); 9 (45%) specimens were laboratory-confirmed for CDI (8.6 cases/10,000 LTCF resident-days).
Conclusions: New-onset diarrhea was common among inpatients and LTCF residents. Only one-third of patients with new-onset diarrhea had a stool specimen collected and tested for C. difficile-indicative of a potential CDI underdiagnosis-although, further studies are needed to confirm the extent of CDI underdiagnosis.

is a protozoan parasite that typically causes self-limited diarrhea in healthy individuals but can result in prolonged, severe illness in those who are immunocompromised. While this infection is well recognized in HIV-positive patients, it is less frequently reported in individuals with lupus nephritis on immunosuppressive therapy. We report the case of a 27-year-old man with biopsy-confirmed class IV lupus nephritis who was receiving cyclophosphamide and corticosteroids.

The diagnosis of new onset autoimmune diseases in kidney transplant patients is challenging with new onset SLE after kidney transplant being a rare condition, with very few cases reported. We describe a case of a kidney transplant patient who, after 9 years presented with intermittent fever and diarrhea, inflammatory migratory polyarthralgia, myalgias, cramps and an erythematous rash in the cleavage area. Immunological testing revealed positive antinuclear antibodies (ANA 1:640), anti-double-stranded DNA antibodies (53.

Background: Systemic lupus erythematosus is a multisystem inflammatory disease with a broad range of clinical and serologic presentations. The heterogeneity of presentation poses diagnostic challenges for the clinician, and a high index of suspicion is required. Classification systems exist for both clinical and immunologic criteria; however, they may lack sensitivity in assisting with diagnosis of atypical presentations.

Background: Food protein-induced enterocolitis syndrome (FPIES) is a non-IgE-mediated reaction to food, well known in the allergy and immunology literature, that presents with delayed gastrointestinal symptoms. It is classically diagnosed in children and infants; however, new-onset cases have recently been identified in the adult population.
Summary: There is a paucity of literature on FPIES in the gastroenterology literature.

Background: To characterize Clostridioides difficile isolates identified during a prospective multi-hospital population-based study of C. difficile infection (CDI).
Methods: Between December 2018 and March 2020, inpatients ≥50 years of age with new-onset diarrhea in nine Tokyo hospitals were investigated for CDI.
