Some evidence suggests a reduction in clinical and surgical recurrence after mesenteric resection in Crohn's Disease (CD). The aim of the REsection of the MEsentery StuDY (Remedy) was to assess whether mesenteric removal during surgery for ileocolic CD has an impact in terms of postoperative complications, endoscopic and ultrasonographic recurrences, and long-term surgical recurrence. Among the 326 patients undergoing primary resection between 2009 and 2019 in two referral centers, in 204 (62%) the mesentery was resected (Group A) and in 122 (38%) it was retained (Group B). Median follow-up was 4.7 ± 3 years. Groups were similar in the peri-operative course. Endoscopic and ultrasonographic recurrences were 44.6% and 40.4% in Group A, and 46.7% and 41.2% in Group B, respectively, without statistically significant differences. The five-year time-to-event estimates, compared with the Log-rank test, were 3% and 4% for normal or thickened mesentery ( = 0.6), 2.8% and 4% for resection or sparing of the mesentery ( = 0.6), and 1.7% and 5.4% in patients treated with biological or immunosuppressants versus other adjuvant therapy ( = 0.02). In Cox's model, perforating behavior was a risk factor, and biological or immunosuppressant adjuvant therapy protective for surgical recurrence. The resection of the mesentery does not seem to reduce endoscopic and ultrasonographic recurrences, and the five-year recurrence rate.

A 12-week-old intact male golden retriever dog was referred to our clinic with a history of recurrent diarrhea and rectal prolapse and because of a suspected intussusception. An abdominal ultrasound was conducted to confirm the suspicion of an intussusception. An exploratory laparotomy identified a jejuno-ileo-cecal-colic intussusception that was manually reduced.

Aim: Controversy exists over whether surgical technique can reduce recurrence following Crohn's resection. This study compares the rate of endoscopic recurrence after different approaches to mesenteric excision (extended/close) and anastomosis (Kono-S/standard of care) in adult patients undergoing ileocolic resection for primary or recurrent Crohn's disease.
Method: MEErKAT is a UK multicentre, 2 × 2 factorial, randomised, controlled, open-label superiority trial where participants (target sample size = 308) are blinded and centrally randomised (1:1:1:1) to one of four groups: (1) Kono-S + extended mesenteric resection.

Objective: To evaluate the value of mesenteric approach for borderline resectable pancreatic head cancer(BRPHC).
Methods: According to the inclusion and exclusion criteria, the data of 203 patients with borderline resectable pancreatic head cancer(BRPHC) who underwent radical pancreatoduodenectomy in our center from January 2011 to December 2023 were retrospectively analyzed. After detecting the superior mesenteric artery in infracolic compartment to judge that there was vascular invasion but it met the borderline resection criteria, the blood supply was cut off first, then the tissues and organs were resected.

Castleman disease (CD) is a group of rare lymphoproliferative disorders characterized by shared histopathological features but distinct clinical entities, broadly classified into unicentric Castleman disease (UCD) and multicentric Castleman disease (MCD). UCD involves a single anatomical site and typically follows a benign clinical course, whereas MCD affects multiple lymph node stations and is associated with systemic symptoms and a more complex therapeutic approach. The disease is poorly understood, and the difficulty in reaching a diagnosis is well noted in the literature.
