Unlabelled: Rapid-onset obesity with hypothalamic dysfunction, hypoventilation, and autonomic dysregulation (ROHHAD) is a rare cause of syndromic obesity with risk of cardiorespiratory arrest and neural crest tumor. No ROHHAD-specific genetic test exists at present. Rapid weight gain of 20-30 pounds, typically between ages 2-7 years in an otherwise healthy child, followed by multiple endocrine abnormalities herald the ROHHAD phenotype. Vigilant monitoring for asleep hypoventilation (and later awake) is mandatory as hypoventilation and altered control of breathing can emerge rapidly, necessitating artificial ventilation as life support. Recurrent hypoxemia may lead to and/or right ventricular hypertrophy. Autonomic dysregulation is variably manifest. Here we describe the disease onset with "unfolding" of the phenotype in a child with ROHHAD, demonstrating the presentation complexity, need for a well-synchronized team approach, and optimized management that led to notable improvement ("refolding") in many aspects of the child's ROHHAD phenotype over 10 years of care.

Background And Objectives: Years before diagnosis of Parkinson disease (PD), dementia with Lewy bodies (DLB), or multiple system atrophy (MSA), mild prodromal manifestations can be detected. Longitudinal follow-up of people with prodromal synucleinopathy, particularly idiopathic/isolated REM sleep behavior disorder (iRBD), enables in-depth clinical phenotyping of early disease, which could facilitate stratification for clinical trials, provide the definition of appropriate end points, or predict phenoconversion more precisely. The aim of this study was to update and expand on previous studies assessing clinical evolution from iRBD to clinically diagnosed disease, up to 14 years before diagnosis.

Multiple system atrophy (MSA) is a progressive, adult-onset neurodegenerative disorder involving autonomic failure, cerebellar ataxia, and parkinsonism. Patients often require invasive interventions, such as gastrostomy or tracheostomy, and sudden death is common. This study aimed to elucidate patterns of invasive treatment and identify risk factors for tracheostomy or sudden death within 5 years of onset.

Speech disorders differ between Parkinson's disease (PD) and multiple system atrophy (MSA), but studies focusing on group differences based on syllables or including cerebellar ataxia (CA) are lacking until now. This cross-sectional study aimed to analyze syllable-based speech characteristics in patients with PD, MSA, and CA, as well as healthy controls, to determine their diagnostic utility. Purpose Of Review: This review describes and summarizes the relationships between psychosocial stress and cardiovascular disease risk in women and offers strategies and recommendations to improve health outcomes.
Recent Findings: Psychosocial stress plays a pivotal role in the cardiovascular health of women, acting both as a precipitant and an outcome of CVD. As a precipitant, chronic stressors such as caregiving responsibilities, socioeconomic adversity, intimate partner violence, and gendered barriers to healthcare can exacerbate stress-related CVD risk factors which in turn predispose to upregulation of inflammatory factors.
