Purpose Of Review: Infections can result in serious complications in solid organ transplant (SOT) patients. The need to remain up to date on recommendations on screening, vaccinations, and chemoprophylaxis is paramount in the management of SOT patients. The goal of this review is to provide an overview of current recommendations for the prevention of infections and optimization of vaccinations from the pretransplant through posttransplant periods.
Recent Findings: There is an emphasis on thorough pretransplant evaluation to guide clinicians and pretransplant testing based on epidemiological and endemic risk factors. Additionally, recent studies on vaccine safety and efficacy of newer vaccine formulations in SOT recipients are addressed.
Summary: This review provides insight on updated recommendations for pretransplant screening, new data on vaccine optimization in SOT recipients and posttransplant prophylaxis. Further research is needed in order to improve preventive measures including screening tests, vaccines, and chemoprophylaxis.

Extracorporeal photopheresis (ECP) is a therapeutic intervention for modulating immune responses using an autologous apoptotic cell-based product, known as a photopheresate. The process of generating photopheresates offers attractive possibilities for manipulating distinct leukocyte subsets to either augment or dampen immune responses, depending on the disease context. This review discusses current uses of ECP as a cell-based therapy and introduces possible strategies to enhance the potency of photopheresates.

However, modern maintenance immunosuppression is accompanied by serious side effects, such as an increased risk of infection and malignancies. Cytomegalovirus (CMV) infection poses significant challenges in solid organ transplant (SOT) recipients, impacting graft outcomes, morbidity, and in some cases survival. The ESOT CMV Workshop 2023 convened European experts to discuss current practices and advances in the management of CMV with the aim of improving the quality of life of transplant recipients. Discussions covered crucial areas such as preventive strategies, diagnostic challenges, therapeutic approaches, and the role of cell-mediated immunity (CMI) monitoring.

T-cell prolymphocytic leukaemia (T-PLL) is an aggressive and rare post-thymic T cell malignancy, highly refractory to conventional cytotoxic chemotherapeutics. While extranodal involvement is common, solid organ invasion is rare. We present the case of a 76-year-old man who developed acute renal failure secondary to T-PLL renal infiltration.

Background: Immune checkpoint inhibitors (ICIs) are effective against solid tumors but can trigger immune-related adverse events (irAEs), including adrenal insufficiency (AI). Methods: We conducted a retrospective analysis of 46 patients diagnosed with ICI-AI at a single center (May 2019-July 2024) and reviewed clinical trials/real-world studies on ICI-AI.
