Article Abstract
We report a Delphi Consensus modification and first validation study of the Autism Diagnostic Observation Schedule - 2 with deaf children and young people (ADOS-2 Deaf adaptation). Validation included 122 deaf participants (aged 2-18 years), 63 with an Autism Spectrum Disorder (ASD). This was compared to a National Institute for Health and Clinical Excellence (NICE) guideline standard clinical assessment by blinded independent specialist clinicians. Results showed overall sensitivity 73% (95%CI 60%, 83%); specificity 71% (95%CI 58%, 82%), and for the more common modules 1-3 (combined as in previous studies) sensitivity 79% (95% CI 65-89%); specificity 79% (95% CI 66-89%) suggesting this instrument will be a helpful addition for use with deaf children and young people.

In the recent two decades it became possible to compensate severe-to-profound hearing loss using cochlear implants (CIs). The data from implanted children demonstrate that hearing and language acquisition is well-possible within an early critical period of 3 years, however, the earlier the access to sound is provided, the better outcomes can be expected. While the clinical priority is providing deaf and hard of hearing children with access to spoken language through hearing aids and CIs as early as possible, for most deaf children this access is currently in the second or third year of life.

Early Childhood Development is a key national priority in South Africa which has developed the Early Learning Outcome Measure (ELOM 4&5) specifically designed to measure the progress of 4- and 5-year-old children across 5 domains of early childhood development. This age-validated, population-standardised instrument has been shown to have measurement equivalence and lack of bias across South Africa's 11 official spoken languages. In 2023, South African Sign Language was formally recognised as 12th official language of South Africa, but no ELOM (4&5) exists in SASL despite over 6,000 deaf children being born annually.

An algorithm has been developed to accompany adolescents at the stages of determining indications and postoperative rehabilitation for sequential bilateral cochlear implantation with a long interval between operations, minimizing the risk of failure to use a cochlear implant (CI) in the second ear (CI2) and ensuring its effective use. This study examined the effectiveness of the explicit, evidence-based vocabulary intervention, Text Talk, for deaf children attending mainstream spoken language education settings during the early years of school. The Text Talk program is designed to guide teachers' Tier Two vocabulary instruction in the context of read-aloud storybooks.
